If you are an attorney whose client has been declared indigent for due process costs, and you have not signed the Attorney Agreement (Indigent for Costs), please:

• Print 2 copies,
• Sign both copies in blue ink, date and
• Return both copies of the Agreement to the Justice Administrative Commission, P.O. Box 1654, Tallahassee, FL 32302.

The Justice Administrative Commission will then execute the Agreement and return one copy to you for your records.
