INSTRUCTIONS FOR ATTORNEY AGREEMENT (REGISTRY)

  2009-2010 Standard Attorney Agreement

If you have been selected by the Chief Judge for the conflict registry and you have not signed the 2009-2010 Agreement for Attorney Services, please:

• Click here to download the 2009-2010 Standard Attorney Agreement,
• Please type information directly into the shaded fields. Please assure that all information entered is accurate and complete.
• Print 2 copies,
• Sign both copies in blue ink, date and
• Return both copies of the Agreement to the Justice Administrative Commission, P.O. Box 1654, Tallahassee, FL 32302.
• If you have not already submitted an IRS W-9 form, or if you have a change of mailing address and/or Tax ID number, please click here. JAC cannot execute the 2009-2010 Standard Attorney Agreement if it does not have an IRS W-9 form on file.

The Justice Administrative Commission will then execute the Agreement and return one copy to you for your records.

(If you are an attorney who has not been approved by the Chief Judge for the registry, you may not execute this contract. This contract is intended for use only by those attorneys selected for the registry by the Chief Judge).
