Pursuant to Chapter 119, Florida Statutes, requests for public records may be submitted in person, by mail, online submission form, email or telephonically. A public records request does not have to be in writing as a prerequisite for completing a public records request. Please feel free to contact this office in writing or by phone.

Please note, if you wish to use the online submission form, no fields are required; however, if you choose to make a public records request using this form, please note email addresses are public records. If you do not want your email address released in response to a public record request, please use these accessible versions (English and Spanish) of the Public Records Request form instead and deliver the form in person or by postal mail.

For complete anonymity, please deliver the form in person to this office.
